What We Do

Empowering Adults with Disabilities Through Comprehensive Services

At LifeROOTS, Inc., we are dedicated to serving adults aged 18+ by addressing their unique needs and empowering them to achieve independence and self-sufficiency. Through our comprehensive Service Coordination, we guide individuals from orientation and intake, connecting them to programs and services tailored to their personal goals. Our Service Coordinators develop personalized support plans, maintain detailed case files, and ensure every individual’s concerns and interests are addressed for holistic care.

LifeROOTS collaborates with the Developmental Disabilities Supports Division (DDSD) to provide access to critical Medicaid waiver programs. These include the Developmental Disabilities Waiver (Traditional Waiver), Medically Fragile Waiver (Traditional Waiver), Mi Via Self-Directed Waiver, and the Supports Waiver. DDSD’s Intake and Eligibility Bureau manages the Central Registry for individuals applying for and waiting for services. In addition to these programs, DDSD offers several State General Funded Services to ensure individuals receive comprehensive support.

As a proud vendor with the Division of Vocational Rehabilitation (DVR), we offer a wide range of Job Development and Job Coaching services to support adults through every step of the employment process. From resume writing and interview preparation to securing the ideal job, our team ensures success. Once employed, our Short-Term Job Coaching provides ongoing support to help individuals excel in their roles and thrive in their journey toward self-sufficiency.

LifeROOTS also provides meaningful employment opportunities through Contract Support Services designed specifically for adults with disabilities. These include custodial contracts within the Albuquerque area and positions with local businesses that employ individuals with disabilities. As we expand this program, we remain committed to increasing opportunities for our clients and fostering inclusive work environments.

To ensure long-term success, we implement Fade and Follow-Up Plans for adults in our programs and their employers. Through regular check-ins, we provide ongoing support, ensuring a strong and productive working relationship for all parties involved.

Supported Employment:

Supported Employment – Empowering individuals with various disabilities to thrive in the workplace through personalized support that combines tailored job matching to align talents with the right roles, hands-on coaching to build essential skills, and ongoing support to ensure long-term success in diverse, inclusive environments.

Job Development – From resume writing to interview preparation, we help candidates find the right job for them.

Job Coaching & Support:

Job Coaching – Provides both short-term and long-term support to empower individuals in acquiring new job skills, mastering tasks, and enhancing their overall workplace performance.

Fade Plan – Gradual reduction of support as individuals become comfortable and skilled within their new roles.

Check-ins – Regularly scheduled interactions between the Job Coach and the individual, designed to review progress, address challenges, provide feedback, and set actionable goals to ensure continued growth and success in the workplace.

Service
Coordination:

Comprehensive support from the initial intake to maintaining individualized plan(s).
Coordination with LifeROOTS and other agencies to ensure meaningful outcomes for each individual.
Creating customized Community Integrated Employment (CIE) plans in collaboration with the individual’s support team.

Eligibility

Our services are available to individuals from 18+ who have various disabilities. We work with the individual to determine eligibility and develop a plan tailored to each of their needs.
